Bus Information and Registration - Updated July 2022

District 64 will be once again providing bus service to our school this year. The attached Google form is only a registration form and will let the district determine if you qualify for service. Bus service is limited to students whose families live within the boundaries of Park Ridge/Niles school district. You must live 1.5 miles from school and/or have a hazardous crossing (railroad crossing or busy intersection) in order to qualify.

Bus service is for Kindergarteners through Eighth graders. Preschoolers and Prekindergarteners do not qualify for service.

Volunteer Compliance Checklist - Protecting God's Children 
We look forward to seeing more of you at school this year in the capacity of volunteering. Per the Archdiocese of Chicago and the State of Illinois requirements, there are mandatory items that need to be completed before we can have anyone work with children. Therefore, an explanation of the requirements are listed here.

Our online system called FACTS will help manage tuition accounts as well as all applications and enrollments. Within FACTS, you will be able to submit all required forms, with the exception of the health forms. You will turn in your required health forms directly to the school office. 

The following forms are required for the 2022-2023 school year and will be submitted directly to the school office. 

Physical Examination Information and Form   Must be submitted by August 1, 2022
*Required for Preschool, Prekindergarten (if new to SPC), Kindergarten and 6th Grade; all new and transfer students in any grade.

Dental Examination Information and Form  Must be submitted by August 1, 2022
*Required for Kindergarten, 2nd Grade, and 6th Grade; all new and transfer students in any grade.

Eye Examination Information and Form  Must be submitted by August 1, 2022
*Required for Kindergarten; all new and transfer students in any grade.
