These items will be required at the time of registration:

1. The child’s ORIGINAL legal birth certificate (NOT a copy and NOT a hospital certificate) as required by the Illinois State Board of Education
   
   
2. A copy of the child’s baptismal certificate if the child was not baptized at St. Paul of the Cross Church. If the child was baptized at St. Paul, we do not need a certificate, but we do need the date of the baptism.
   
   
3. The child’s social security number
   
   
4. Your parish envelope number (the 3 or 4 digit number on the far left side of your envelope)
   
   
5. Registration fee as listed on Registration Information Page **Refundable ONLY if St. Paul of the Cross School does not accept the child(ren).
   
   
6. A copy of the child’s latest report card and/or a copy of the latest standardized test scores if registering for grades 1 – 8
